Download and Install the Module
You can download Version 2.3 from www.xoopsaddons.org. Installation is similar to every other XOOPS module.
Create a New Form
In the module's administration area, click on the icon to create a new form - enter the form name and select "More than one per user" for the "How many entries are allowed for this form?" option. Press the Submit button.
Add Elements to the Form
The first thing I add to every form is an introduction - the purpose of the form and how to use it, along with other ways to contact us. As I have said before, your web site is a way to connect with people - give them as many ways to connect as you have available. Include phone numbers and street addresses somewhere on the form (I usually put the phone number in the introduction and the street address in the last field).
Here's a list of fields, their field types and an explanation of the content for those fields:

How can we pray for you?: Radio buttons (default selection = Privately)
Requests for private prayers will be sent to our pastoral team, and they will add you to their personal prayer list.
Prayer chain requests will be sent to members of our Prayer Team, and if you have indicated we may pray publicly for you, your request will be added to the prayers of the church offered during our worship services.
Would you like someone to contact you?: Simple yes/no radio buttons (no default selection)
Some will want follow-up care and contact about their request. Give them the opportunity to connect with someone on your prayer team.
Your Name: Text box (default = {NAME})
This field is optional because some will want to remain anonymous.
Your Email Address: Text box (default = {EMAIL})
Another optional field provided as a means to follow-up and contact the requestor.
Your Phone Number: Text box

Assign Permissions to the Form
From the Formulize administration page, click the icon to Modify form access permissions. Select all the groups you have created for your site and just your new prayer form, then click the 'Show these permissions' button at the bottom of the page. There are a lot of options, but most of them are self-explanatory. There are tooltips that give you more information when you hover over the options - a cool new feature!
- Every group should have the ability to view the form and add their own entry.
- Your prayer team group should have the ability to view group scope and global scope
- You will also want to allow publishing group reports and global reports for your prayer team and site maintainers
Select Fields for the Default View
Back on the form index page, select the icon with the @ symbol to modify the form's settings. Here you will find the options of which field data to display by default when viewing the entries. I've set 'Please pray for', 'Details' and 'How can we pray for you?' as the default fields in our form. You may want to have 'Would you like to be contacted?' as one of your options. This version also gives you the options to display some fields that are automatically populated for each entry - creation date, last modification date, user making the entry, last user to modify the entry and the email address of the person making the entry(they'll need to be signed in for this). Save your choices by pressing the 'Submit' button.
